Section 01 • Scope & Legal Authorities

Scope of Data Processing & Advertising Platform Attribution

Glamorise acts as data controller for contact information submitted through our website and private client records generated during cosmetic appointments at 400 South LaSalle Street.

Our web properties run active advertising campaigns on Google Ads, Microsoft Advertising, and Meta Ads. These platforms deploy click identifiers including Google Click ID (gclid), Microsoft Click ID (msclkid), and Meta Click ID (fbclid) to measure conversions when visitors arrive from search or display placements. Under Google Consent Mode v2, all measurement storage (ad_storage, ad_user_data, ad_personalization, and analytics_storage) is held denied by default until affirmative consent is given, and reverted immediately when declined.

For information on Microsoft’s independent data processing practices, review the Microsoft Privacy Statement.

Section 02 • Available Statutory Rights

Categories of Data Subject Requests Supported

1. Right to Inspect & Access (DSAR Access)

Receive a structured machine-readable export (JSON or CSV format) comprising your full name, email, phone number, booked service types (such as Botox units, dermal fillers, HydraFacial, laser hair removal, CoolSculpting, Morpheus8, or microneedling), session timestamps, and communications history.

2. Right to Rectification & Correction

Update out-of-date mailing addresses, revised phone numbers, or administrative contact preferences across our active booking records.

3. Right to Erasure & Personal Data Deletion

Request full permanent deletion of marketing contacts and digital inquiry submissions. Please note: State of Illinois cosmetology licensing rules and tax accounting standards mandate that procedural consent forms and financial ledgers must be retained for seven years from the session date, after which they are securely shredded.

4. Advertising Opt-Out & Tracking Revocation

Revoke all cookie consent immediately using our in-browser preference selector or submit an inquiry to purge server-side conversion tags linked to your digital identity.

Section 03 • Verification Protocol

Identity Verification & Security Measures

To safeguard confidential personal records against unauthorized disclosure, Glamorise verifies identity prior to processing any DSAR submission. We match the full legal name, telephone number, and email address provided in your request against records on file. If necessary, a single-use verification token will be transmitted to the confirmed email address.

Authorized agents submitting on behalf of an Illinois consumer must provide a signed power of attorney or notarized client authorization before procedural records will be released.

Section 04 • Record Storage Security

Physical & Digital Storage Architecture

Procedural records and signed non-medical acknowledgments are stored in locked, fire-rated steel cabinets within our private administrative office at 400 South LaSalle Street. Digital booking logs and zofi892.php inquiry receipts are encrypted using TLS 1.3 in transit and AES-256 at rest. Access is strictly restricted to our solo licensed aesthetician.

Secure locked records archive for client consent and compliance files

Mandatory Non-Medical Declaration

Glamorise provides non-medical aesthetic and personal care sessions. These cosmetic services do not diagnose, treat, cure, or prevent any disease, and do not replace consultation with a qualified medical doctor.

Data subject access requests do not alter medical records held by external healthcare providers, dermatologists, or physicians from whom clients have obtained procedural clearance.

Practice Credentials Solo licensed cosmetologist in aesthetic practice since 2019 with 120 logged sessions in Chicago, Illinois.
